On February 22, 2000, a Notice of Violation was issued for a Severity Level III problem based on (1) failure to comply with the technical specification for operating the emergency diesel generator; (2) failure to establish adequate alarm response instructions; (3) failure to review a work package, and (4) failure to initiate condition reports for nonconformances.
